Abstract

META ANALYSIS OF ANGIOTENSIN-II EFFECTS WITH VASOPRESSIN AND NOR-EPINEPHRINE IN THE TREATMENT OF DISTRIBUTIVE SHOCK



Objective: This review focuses on the safety and efficacy parameters of concomitant use of Angiotensin II (Giapreza), Vasopressin, and norepinephrine in treating distributive shock.

Methodology: The review data is generated by systematic searches in the PubMed/Micromedex database to acquire studies about the safety and efficacy of norepinephrine (NE), vasopressin, and Angiotensin-II (Giapreza). The safety analysis included parameters such as mortality rate, causality, and the extent of adverse events related to the vasopressors and their usage in special populations. The Efficacy analysis compared the data pertaining to Mean Arterial Pressure control by NE, vasopressin, and Giapreza.


Outcome: On analysing the various pharmacokinetic data, Giapreza requires a very short time to reach steady-state plasma concentration, indicating a quick onset of action with a half-life of less than a minute with low toxicity profile. Vasopressin, on the other hand produces a large volume of distribution due to its inability to bind to plasma proteins. NE requires an suitable time to reach the steady-state concentration and a low half-life, which makes it as a standard drug of choice towards distributive shock. Angiotensin-II shows promising results in comparison with a placebo, but the mortality rate makes it a questionable drug on the safety aspect. Further studies involving the safety and concomitant therapy, including other vasopressors, are required to demonstrate the capabilities of Giapreza.
